In a nutshell
● The Ministry of Health has published a lifelong vaccination schedule for 2024 with two important innovations.
● On the one hand, the inclusion of rotavirus vaccination for all infants from 6 weeks of age.
● And, on the other hand, simplifying the HPV vaccination schedule by offering only two doses to people (healthy or with certain risk factors) aged 15 to 25 years. A three-dose schedule is maintained in immunocompromised people of any age, starting at 9 years of age.
● The Ministry of Health is postponing until next year the final transition to a single-dose HPV vaccination regimen for adolescents.
● Similarly, the review of Covid vaccination and RSV immunization has been delayed until the autumn.

Introduction

By tradition, the Ministry of Health (through the Interterritorial Council of the SNS -SNGS-) updates its “lifelong vaccination schedule“.

New developments in 2023 included meningococcal B vaccination in the first year of life, influenza vaccination for infants and children aged 6–59 months, and the expansion of human papillomavirus (HPV) vaccination to adolescent males.

News and other noteworthy facts about the 2024 calendar

The news is this:

  • The main novelty is the inclusion rotavirus vaccination in children from 6 weeks of age. All CS. AA. They were supposed to include this vaccination by the end of 2025.
  • An equally remarkable fact is that in 2024 the one-dose HPV vaccination schedule will not be adopted (as was decided in the same February 2024 in Andalusia), but the vaccination schedule will be simplified to one dose. two dose regimen in healthy adolescents under 18 years of age. Likewise, the possible introduction of a single dose regimen has been delayed until next year.

2024 calendar does not include immunization against RSV And Covid vaccination (currently active). Regarding these vaccinations, the ministry adds: “Recommendations for vaccination/immunization against COVID-19 and RSV will be assessed at the end of the 2023-2024 season.” That is, recommendations already implemented in the fall of 2023 are preserved, and new data and analysis are expected for the recommendations for the fall of 2024.

It should be noted that the new vaccines included in the 2023 immunization schedule and which should have been included before the end of 2024 (meningococcus B in infants and HPV in adolescent males) are already present in all SCs. AA. until the end of 2023.

Vaccination against rotavirus

According to the Ministry of Health calendar for 2024:

  • Vaccination against RV is given to children from 6 weeks of age.
  • In infants below the gestational age specifications (≥25 weeks in RV5 (RotaTeq); ≥27 weeks in RV1 (Rotarix)), they can be administered with the informed consent of the parent or legal guardian.
  • Vaccination will be included in all SCs. AA. until the end of 2025.
  • No preference is indicated for either of the two available vaccines.

Technical document on rotavirus vaccination

  • Ministry of Health, Public Health Commission of the Interterritorial Council of the National Health System, January 2024. Rotavirus Vaccination Working Group Vaccination programs and registry presentation. Recommendations for vaccination against rotavirus.

This is a 67-page document. In paragraph 10 of recommendationsthe following is said:

  • Taking key considerations into account, RV vaccination will reduce the incidence of acute gastroenteritis in the pediatric population. However, at the current retail price of RV vaccines (93.66 €/dose RV1 and 69.50 €/dose RV5), the criterion for the implementation of this vaccination program is not met. From a social point of view, the introduction of vaccination against RV is effective with a price for the complete regimen at RV1 equal to or less than 118.20 euros and at RV5 equal to 115.50 euros.
  • If vaccination is effective:
  1. Vaccination against RV is recommended for children from 6 weeks of age.
  2. Either of the two available vaccines can be used. In infants with a gestational age below that indicated in the technical descriptions (≥25 weeks in RV5; ≥27 weeks in RV1), they can be prescribed with the informed consent of the parents or legal guardians.
  3. It is recommended to establish a RV disease surveillance system that includes ongoing microbiological characterization to assess the impact of vaccination on the disease and identify possible changes in circulating RV genotypes.
  • These recommendations will be implemented in all COPs. AA. until the end of 2025.

HPV vaccination in adolescents

According to the Ministry of Health calendar for 2024:

  • Vaccination at age 12: Boys and girls will receive 2 doses at least 6 months apart.
  • Vaccination after 12 and up to 18 years of age: unvaccinated or partially vaccinated women under 18 years of age and men are vaccinated with 2 doses from the date of introduction into the vaccination calendar with an interval of at least 6 months.

White Paper on HPV Vaccination

The 19-page PDF whitepaper, in addition to what was previously mentioned, states that it is proposed to switch from a 3-dose regimen to a 2-dose regimen in people aged 15 to 25 years (inclusive). For people 26 years of age and older, and for people of any age over 9 years with immunosuppression, the recommendation for a 3-dose schedule (0, 1-2, and 6 months) remains the same (p. 16). ):

  • Unvaccinated people with certain risk situations. Regimen of 2 doses with an interval of at least 6 months:
    • Men who have sexual relations with men, up to 25 years of age inclusive.
    • The situation with prostitution up to 25 years of age (inclusive).
  • For people with immunosuppression and up to and including the age of 45 years, a 3-dose schedule (0, 1-2 and 6 months) is always recommended, regardless of the age at which vaccination begins, including:
    • WHIM syndrome (IDP): vaccine covering types 6 and 11.
    • HIV infection.
    • Transplantation of a solid organ or hematopoietic precursor.
  • Women, regardless of age, who have received any treatment for high-grade cervical intraepithelial lesions (CIN2+): 3-dose regimen (0, 1-2 and 6 months).
